IMPORTANT: In addition to providing registration, every team or single must complete a payment form if not signing up online. These forms must be completed and mailed to Pat Lavery at 8317 Coyote Creek Dr, Peoria, IL 61607 by May 15, 2024 to guarantee your spot in the event.
